At Fivestar Health Care LLC, we provide compassionate, in-home care through Georgia’s Elderly and Disabled Waiver Program (EDWP). This program helps older adults and individuals with physical disabilities remain safely and comfortably in their own homes and communities — rather than in nursing homes or institutional settings.

🏡 Personal Support Services 

Our Personal Support Services assist individuals with daily living activities such as bathing, dressing, grooming, meal preparation, light housekeeping, and medication reminders. We focus on promoting independence, comfort, and dignity while providing the care each individual deserves.

💞 Extended Personal Support Services

Extended Personal Support Services offer additional hours of assistance for individuals who require more intensive care. This ensures continuous support for those with complex needs or limited mobility — helping them maintain quality of life in the comfort of their homes.

👪 Structured Family Caregiver (SFC)

The Structured Family Caregiver Program allows qualified family members to provide daily care to their loved ones at home while receiving financial and professional support from Fivestar Health Care LLC. We provide ongoing training, guidance, and case management to ensure caregivers have the tools they need to deliver exceptional care.
